What is case-management?

Twenty10 works from a client focused case management model. This means that our case workers will help you work towards the goals that you want to achieve. Our case workers can help you identify your aims, provide advocacy, help you get to appointments and provide brokerage to assist you in meeting your goals.

HPIC is the telephone information and referral service that refers people who are homeless, or at risk of homelessness, to the services that may assist them. The service is provided by the City of Sydney.
